Palliative and End of Life Care
With an aging population, more complicated presentations and more chronic illness, patients need their conditions better managed now more than ever before. We are able to make routine contacts with this cohort of patient to ensure you do not miss opportunities to allow your patients a more dignified end to their life. We will involve patients and families in decision making and create individual care plans which are then sent back to the patients named GP to ensure the plan is enacted.
Personal Approach
All our clinicians have specific experience in dealing with End of Life Care. They know the importance of a personal approach and understand the complex emotions that surround this journey. We believe our support needs to be directed to beyond the patient and include close family members in what is a time of emotional distress for all.

My husband was diagnosed with a terminal illness in the summer. By December, he had deteriorated rapidly. I struggled to understand how he could be unwell so quickly. My GP was difficult to reach and I did not understand the complicated language of the hospital consultants.
My GP then referred me to the Primary and Urgent Care Alliance and they made sure we both understood what was happening and what to expect. I received a call every few days and they had thought of things that I had not considered before. They ensured my husband was referred to the district nurses, palliative teams and made sure all of the medication was ready for when he may need them.
I could have got through this time without the Alliance and my GP working together.
