Care Home Management

Our Care Home team can offer a visiting service specifically targeting patients who are in residential care.

The team offer face to face reviews to include frailty, dementia, medication side effects, advance care planning and palliative care.

Ensuring the patient receives one to one care with clear and concise consultations sent back to the named GP ensures continuity of care.

Personal Approach

All our clinicians have specific experience in dealing with End of Life Care. They know the importance of a personal approach and understand the complex emotions that surround this journey. We believe our support needs to be directed to beyond the patient and include close family members in what is a time of emotional distress for all.
